Certidão de tempo de serviço militar

The Department of Veterans Affairs (VA) website indicates that there are several types of military records. Please note that the "Report of Separation" is just one of them. The "Report of Separation" is another name for the Certificate of Release or Discharge from Active Duty, as already mentioned in my answer.

Elaborating on my response to you a little further, I would say that "DD Form 214" does not belong in the translation, since there is no such form in Brazil. The name of the document does make sense, though. It is the counterpart to the "Certidão de Tempo de Serviço Militar" in the sense that both contain information as to the amount of time in military service. I don't see why not use an English equivalent. A good rule of thumb would be to put the PT name in brackets, something like this: Certificate of Release or Discharge From Active Duty ["Certidão de Tempo de Serviço Militar" in Brazilian Portuguese]. Antes que alguém interprete mal a minha tradução, convém esclarecer que ela não equivale ao Certificado de Dispensa de Incorporação (CDI). O conceito de CDI não existe no inglês americano, uma vez que o serviço militar não é obrigatório nos EUA.

“O Certificado de Dispensa de Incorporação (CDI) é um documento que comprova que um determinado cidadão brasileiro se alistou em alguma força armada, porém, foi dispensado do serviço militar, tendo em vista suas situações peculiares ou por excederem as possibilidades de incorporação existentes.” https://rb.gy/751rg

Já o “Certificate of Release or Discharge from Active Duty (Form DD 214)” é para quem prestou o serviço militar! (“A DD Form 214 proves military service.”)
