OA support I'm finding a modest but convincing number of online instances of "OA" with "Orden de Atención" such as these examples:

"Ahí se hará entrega de una "Orden de Atención" (OA), la que debe ser presentada previa a la atención en la caja del Centro Médico San Joaquín (CMSJ)."

"La sucursal emitirá una Orden de Atención (OA).El trabajador acude con su OA a un prestador médico en convenio con el INP Sector Activo."

So I think this is a medical referral or authorization (in the case of this question it's for an MRI).

I think there are too many possibilities. I'd put "unknown abbreviation".

Could very well be; I've never seen one of these little receipts before. It's small ... it's the size of, and formatted like, a credit card receipt

I believe but can't really find anything to back it up that it is Orden de Atencion. This is tipical of LA countries with somebody who has insurance, they have to have an orden de atención , orden de exámenes auxiliaries, orden de internamiento, etc. Does this fit your context at all?
