I have now checked with a Russian medical professional and my suggestion in the discussion box is correct.
положение does not need to be translated into English as the verb "to be" covers it and the whole phrase should be translated:
"Physical Examination: The patient is alert and mobile."

| Many thanks, Aiherzen! My understanding is that it's about the patient's body position in bed whereas "alert" would be a reference to his mental state. I think that stating that he s mobile would suggest that he can walk or at least stand up but there is no indication of this in the original. |

Discussion
I am struggling to find out precisely what "положение активное", my instinct is that it will just translate as "active" or "mobile", indicating that the patient is able to move on their own.
It might just mean that the patient can move with the help of others (but I don't think this is so likely)
I think the whole phrase should go something like this:
"Physical Examination: The patient is alert and mobile".
