Come abbiamo riferito nella CTP il pz. soffriva di una Sindrome Eosinofila Generalizzata "occulta e indolente" sul piano clinico. A nostro avviso tale sindrome eosinofila, in assenza di tumori, di infezioni, di patologie allergiche (assenza di asma) o di sindromi anafilattoidi o anafilattiche, scaturiva dalla severa esofagite eosinofila da reflusso che nella fattispecie si associava a gastrite e colite eosinofila (c.d. malattia eosinofila esofago-gastro-colica).
Noi riteniamo quindi che l'assunzione del farmaco "paracetamolo" unitamente all'espansione gastrica secondaria all'assunzione abbondante della coca cola (di cui abbiamo notizia solo verbale), abbiano determinato un'esacerbazione del reflusso e dell'irritazione della giunzione esofago-gastrica con richiamo e degranulazione degli esinofili dal circolo prima a livello dell'apparato digerente e poi nel polmone, con conseguente danno vasculo-parenchimale. L'epitelio squamoso dell'esofago contiene infatti recettori per il richiamo e l'attivazione (degranulazione) degli eosinofili.
Secondo letteratura il richiamo e l'attivazione degli eosinofili in corso di esofagite eosinofila e/o di reflusso gastro-esofageo avviene in virtù dell'iperincrezione dell'Eotaxin-3+ e dell'IL-5 da parte rispettivamente dell'epitelio squamoso dell'esofago e dei vasi esofagei allorquando il succo acido gastrico risale nel III° inferiore dell'esogago.
