Un salarié peut ne plus être en mesure d’effectuer certaines tâches liées au poste de travail : montée d’échelles, contact avec un produit chimique particulier par exemple. Si ces situations sont relativement peu fréquentes dans la tenue du poste, une réorganisation mineure du travail peut suffire pour maintenir le salarié à son poste.

there's a lot of ghits for "modified work program" and "modified work offer", but NONE for "post modified work offer", which leads to a reasonable assumption that this is a Post(-injury) Modified Work Offer. Anyway, logically, it's not only the workplace that needs adapting but a whole new package, a new adapted "work offer", would be needed (working hours, breaks ...).
