What bothers me is that the pair isn't "Kassenpatienten / Privatpatienten".

Is there a difference in billing that makes it lucrative to count the "Schein" from Kassenpatienten and merely count the number of heads among private patients?

A "Kassenpatient" has to hand in a paper called "Schein" issued by his "Krankenkasse" to say that the patient is registered with them and they will cover the cost. 1 Schein = 1 Kassenpatient.
