You can still find diastrophic dysplasia in an MRI scan, even if it is congenital....

diastrophic dysplasia

And
If the diagnosis just says
lumbosacral DTD
it is hardly likely to be the doctor's initials as you can't have a diagnosis that just says
lumbosacral
!

The other possibility is that this is a typo for "DDD"
lumbosacral degenerative disc disease
http://www.google.co.uk/search?q=lumbosacral DDD&btnG=Search...
but then the Spanish would be
DDD lumbosacral
this is why you need to put the context in Spanish.

If you say it is not DDD, then it cannot possibly be degenerative disc disease.

Note from asker:
48-year-old man, injured his back falling down steps 1 year ago - MRI done recently for ongoing pain issues shows this disc protrusion - which is why the "post-traumatic disc degeneration" idea appeals to me, even though we may never find DTD on any acronym list. Thanks, Liz... |
not a typo for DDD... it's handwritten |

traumatic disc displacement
According to the context, this could be a description of the lesion. Sometimes an acronym is used in just one hospital... Which would explain why we do not find it in the acronym lists.
