Assuming the barcode is printed using a pad printer e.g. Tampo, a dirty plate or excess pressure on the rubber pad that picks up the ink can cause blurred print which can in turn cause scanning faults.

disagree David Sirett : As far as I know there is no connection between "cliché" and "orifice plate", the latter usually being some variation on "diaphragme".
