My explanations:
Probably some tests missing (or a treatment stopped, as the context is not clear) because between 2 appointments there were 7 days.

Un décalage de consultation could mean either
- an appointment was moved a later date, or
- 2 appointments are not happening at the same time in the case you have various appointments (of different nature indeed). But in this case it will be written "au décalage des consultations" I think.

Without more context and/or a medical knowledge of MM I can't tell more... Hope this helps.

The context is unclear, specifically, whether this is something deliberate, or actually just an accidental cancelled / missed appointment, etc.

Hopefully, the rest of your context may make that clear...
