就诊卡=medical visit card/ medical visit identification card
就诊，等于英文里的visit，也就是每次去看医生的那种“visit"。例如以下连接所示的就诊卡上面，就写着“就诊必带”。因此，为了表达出这种卡的用途，在译文里一定要考虑visit这层含义。

“取“，在这里是“领取”，也就是当检验报告出来以后，凭就诊卡/病历去“领取”检验报告。fetch的意思是"go and bring it ”，在中国的英语课堂上，通常用它来对比bring和take这两个词。于是，就让学生认为，fetch的意思是“去拿来”的意思。但是，在真正的用法上，fetch这个词，当它作为"go and bring ”这个意思来用时，通常是用在对狗的描述上，一只狗从某处出发，把远处的一样东西叼回来，这个动作就叫做fetch. 大约十年前，跟一个英语母语朋友谈话当中，就对他用了fetch这个词，本来想表达的就是“去拿来”的意思，然而，他就给我解释了我上面说到的那些内容。如果对人说“去拿来”，避免产生误解，最好用“go and bring”。
把fetch用在问题提到的那个句子中，抛开上面提到的那层可能产生的歧义，就算没有那种歧义，用fetch也是不妥。“取”在问题的句子中，是“领取/去取”的意思，去取报告，用collect。
因此，那句话可以翻译如下：

（在标语/告示牌等的用语种，其中的are可以省去。）
