This is, IMO, a return envelope addressed to the original institution with the addressee's name and address already printed on it - hence, it is personal. Therefore, all the client has to do is
1) detach the slip
2) put it in the envelope
3) send the envelope, since it has all the information already pre-printed, including the name and address of the client as the sender and the X unit as the
addressee. The X unit may even show through a see-through window in the envelope.

Zdaje mi się ze "personal envelope" w tym przypadku wystepuje w znaczeniu czasem stosowanego przymiotnika "spersonalizowana", więc "spersonalizowana koperta" jak tutaj:

Szanowni Państwo!
W tłumaczeniu programu komputerowego natrafiłam na niezbyt zgrabnie przetłumaczone polecenie. Brzmi ono następująco: „Podaj poniższe informacje dla spersonalizowania instalacji”. Nie wydaje mi się, by istniało takie słowo. Czy angielskie to personalized your installation można zastąpić innym krótkim zwrotem?

Angielskie to personalize można tłumaczyć jako ‘nadać osobisty charakter’, a w kontekście komputerowym raczej jako ‘przystosować do indywidualnych potrzeb’. Są to sformułowania kilkuwyrazowe, niezbyt wygodne w użyciu, toteż zastępuje się je zapożyczeniem wymienionego angielskiego czasownika. W związku z tym w tekstach informatycznych funkcjonuje wyraz spersonalizować, a że nie ma on zastępnika rodzimego, toteż jego przydatność jest oczywista.
