This is a list of demographic data in a clinical study. It is in table format and comes in this context:
sexo: varón/mujer
convivencia: familia origen/familia propia/solo/otros
estado civil: casado/soltero/separado
I have found "conjugal family" here and I think that would work for "familia propia"

Bowing to MediaMatrix' valid point, how about "live with parent(s) or guardian(s) / nuclear family / alone / other living situation"? The only problem is that by trying to cover every possibility, the translation gets longer and longer.

: 'birth family' would be an unfortunate choice of words if the form has to be completed by someone who was adopted or fostered as a child

You are probably right about that one, but then "family or origin" wouldn't work either. Do you have a suggestion for a term that would cover both situations?

For reasons mentioned elsewhere, 'birth family' and 'family of origin' may not be appropriate here.
The standard expressions in (UK) English are those given above, which are all neutral in terms of precise family relationships between the individuals concerned. In particular, orphans or other children in care are usually happy to refer to the people who provided for their childhood needs as 'parents' ('foster parents', etc.) even if they are not members of the same family; likewise, 'in own home' makes no value judgements regarding the form or legality of the convivencia.
