Извештај / Записник о извршеним обавезама
Explanation:
As there are no mentions of any "reconciliation procedure" in the sense of agreeing a solution regarding some dispute related to the contract, the only meaning of "reconciliation" that would make sense is the one in "reconciliation" as used in accounting, which basically means matching payments to invoices, to make sure all invoices have been paid or in other words there are no more "pending obligations".

In short, this "Reconciliation report" would confirm that parties do not owe anything to one another, that ALL mutual contractual obligations have been fulfilled - no future disputes of any kind are no longer possible.

Reconciliation (in accounting) = сравнити примљене уплате и фактуре

so by a rather obvious analogy:

Reconciliation (at the end of a contract) = "свравнити" извршење уговора и уговорне обавезе

neutral  Daryo: translations of these completely "context free" fragments (**fragments** that are repeated all over from one glossary to another) do not prove really anything - no indication whatsoever of context so about as reliable as taking lucky pot shots!
